Membership Form Waiver

To View and Print Member Form - Adobe Acrobat 5.0 or later is required.

For membership signup or renewal, download and then print the Membership Form or contact Betty Schraith.   Send both completed pages with check to Betty Schraith.( Address is on Membership Form and make check payable to NSCM)  The Waiver must be signed and dated as well. Also you can call the Nordic Hotline  (414-299-9639) for the phone numbers of members who can send you an application and answer your questions or attend one of the monthly meetings.

DON'T FORGET TO RENEW YOUR MEMBERSHIP: A valuable benefit of paid-up membership is discounted rates for ski trips. The new club year began on October 1; so current members and life members, it's time to renew your membership. And future members, become a member today! Please return your completed, two-sided membership form and dues as soon as possible to new Membership Chair Betty Schraith. Special Note: Remember to fill out this year's survey question.

Household Membership	$28.00

The membership year runs from October 1 to the following September 30.
